Refer  table in circular

1) To be reviewed annually
2) Whenever there is change in process, new equipment, working condition
3) If an accident happens in a location that had lower risk rating while doing hazard identification
4) If too many incidents are reported even when a control is in existance

Dear All

Is there are any guidelines for how often (time period like three months, six months like)a Risk Assessment shall be reviewed  along with SMP (Safety Management pan) for U/G mines or it is under the discretion of mine management
